This Privacy Policy informs users of the services provided on the iliDent.com website about how iliDent Dental Patient Coordinator (hereinafter: iliDent) processes and transfers personal data voluntarily provided for patient coordination.

iliDent processes the following personal data provided by the patient for the purpose of making contact and patient coordination:
Name, Phone Number, email address, and description of the complaint.

Purpose of Data Processing
The purpose of data processing is to enable iliDent, within the framework of patient coordination, to maintain contact with the person making the inquiry, forward the inquiry to the appropriate dental institution or specialist providing the treatment, and provide assistance with making contact and coordinating further arrangements.

Data Processing Procedure
The data voluntarily provided by the visitor through the contact form on the website is sent by the system via email to the operator of iliDent.

iliDent forwards the email containing the data, in unchanged form, to the dental institution or specialist providing the treatment so that the patient can subsequently receive the appropriate information and care related to their treatment.

iliDent does not store the forwarded personal data in its own system. The role of iliDent is to facilitate contact related to patient coordination and to forward the data to the institution or specialist providing the treatment.

Following the transfer of the data to the institution or specialist providing the treatment, that institution or specialist is responsible for the processing and protection of the data in accordance with its own data processing rules and privacy policy.

Legal Basis for Data Processing
The processing and transfer of personal data is based on the data subject’s consent, pursuant to Article 6(1)(a) of the GDPR.

Consent must be voluntary, specific, informed, and unambiguous. Information relating to health status or complaints may constitute special categories of personal data; therefore, when processing such data, the requirements set out in Article 9 of the GDPR must also be taken into account.

By giving consent, the data subject agrees that iliDent may process the personal data necessary for making contact and patient coordination and may forward such data to the dental institution or specialist providing the treatment for the purpose of further coordination of the treatment.

Rights of the Data Subject
The data subject has the right to request information about the processing of their personal data and, under the conditions set out in the GDPR, may request access to their personal data, its rectification or deletion, or the restriction of its processing.

Where the processing is based on consent, the data subject has the right to withdraw their consent at any time. Withdrawal of consent does not affect the lawfulness of processing carried out before the withdrawal.

The data subject also has the right to object to the processing of their personal data where the applicable conditions are met.
